Enrollment Details: Must enroll in a San Francisco Health Plan and identify the Teen and Young Adult Health Center as their medical home. Must be enrolled in MediCal, San Francisco Health Plan, Health San Francisco, or SF Path.
Enrollment Requirements: Proof of SF Residency, Proof of Income
